Cleveland Clinic’s Primary Care team is dedicated to treating patients as a whole. This team provides comprehensive healthcare for patients and families and focuses on preventing diseases and promoting health and wellness. Services range from preventive care, such as screenings and immunizations, to sick visits and chronic disease management.
By joining the team as an RN, you will provide direct nursing care to patients on a day-to-day basis. In this position, you will use your nursing knowledge to support patients, acting as both an educator and advocate. You will also cross-train to provide coverage for other ambulatory departments within St. Lucie West. A caregiver in this position works Monday-Friday from 8:30AM – 5:00PM.
A caregiver who excels in this role will:
Assess health status by completing a nursing assessment.
Perform telephone triage.
Notify physicians/LIPs of abnormal findings.
Establish a care plan in collaboration with the patient, family, and health care team.
Collaborate with care coordinators to ensure smooth and effective transitions of care.
Assist with specialized diagnostic and therapeutic procedures.
Administer medications and treatments as ordered by physician/LIP and monitor patients for response.
Initiate CPR and other emergency measures.
Provide health education to patients and families.
Develop, evaluate, adapt and document education to learning needs.
Participate in quality initiatives, process improvements, and research activities to improve patient outcomes, patient experience and patient safety.
Maintain professional growth and development in the nursing field and identify annual core competencies.
Minimum qualifications for the ideal future caregiver include:
Graduate from an accredited school of nursing (ADN or BSN program)
Current state licensure as a Registered Nurse (RN)
Basic Life Support (BLS) certification through the American Heart Association (AHA) or the American Red Cross
One year of recent experience as an RN
Will consider LPN, Medical Assistant and/or other unlicensed clinical healthcare experience in substitution of RN experience if applicant is a current Cleveland Clinic caregiver in an ambulatory setting transferring into an RN role but remaining in the same current ambulatory Cleveland Clinic setting OR a current Cleveland Clinic caregiver in a clinical role within the same specialty area transferring into an ambulatory area

Physical Requirements:
Requires full range of motion, manual and finger dexterity and eye-hand coordination.
Requires corrected hearing and vision to normal range.
May requires some exposure to communicable diseases or bodily fluids.
Light Work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly (Constantly: activity or condition exists 2/3 or more of the time) to move objects.
Even though the weight lifted may be only a negligible amount, a job should be rated Light Work: (1) when it requires walking or standing to a significant degree; or (2) when it requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or (3) when the job requires working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of those materials is negligible.
