Position Overview:

Works independently and in collaboration with physicians and other health care providers across the health care continuum to ensure appropriate medication therapy for individual patients within the Sutter system. Performs refill assessments remotely for physician and care team in-baskets. Prescribes medications for refills as well as adjust medications as needed for chronic disease states under a collaborative practice agreement. Is responsible for providing clinical assessment regarding patient home readings and support providers with medication questions.

JOB ACCOUNTABILITIES:

PHARMACY SERVICES
• Develop, enhance, and/or maintain programs/services based on community –specific patient needs, programs/services.
• Develop and maintain effective skills to appropriately assess patients (e.g. physical assessment, medical history, overall health status, quality of life, cultural issues, educational level, language barriers, literacy leave, current symptoms, laboratory results and medication –related problems) to address requests in the Information and Communications Technology (ICT) queue.
• Design comprehensive, evidence –based medication or non-medication health improvement, wellness and /or disease prevention regimens and monitoring plans for patients, document patient care activities appropriately, according to organizations’ s policies and procedures.
• Ability to design a comprehensive evidence-based medication plan, taking into account patient specific factors (laboratory, health status and other electronic health record information) to safely and effectively adjust and refill medications to accomplish ICT pharmacist level work.
• Record patient outcomes according to organization’s policies and procedures, communicate ongoing patient information to other health care professionals.
• Maintain disease and treatment knowledge via current literature and clinical practice guidelines.
• Ability to maintain a patient schedule and complete assessment with defined business policies and procedures.
• Ability to complete tasks (e.g. medication orders, laboratory orders, referrals) within defined business policies and procedures.


CLERICAL:
• Performs general clerical tasks, such as answering phones; updating patient information; or retrieving, entering, and revising patient Electronic Health Records (EHR).
• Prepares and completes accurate documentation related to patient in medical and departmental records as required.


SAFETY
• Participates in quality improvement processes, including updating and revision of safety and procedure manuals and interdisciplinary projects.
• Maintains a clean, neat, and safe working environment by complying with policies, procedures and regulations.
• Protects patients and technicians by adhering to infection-control protocols.
• Appropriately conduct continuous quality improvement (CQI) activities on existing programs/services (e.g. analyze program(s) / service(s) for quality improvement, present and /publish program outcomes and appropriately assess impact and sustainability of program/service.


COMPLIANCE
• Maintains strictest confidence of all patient protected health information (PHI) and protects all PHI from accidental, intentional, or inappropriate disclosure.
• Complies with state and federal drug laws as regulated by the state board of pharmacy.
• Retains certification and completes mandated continuing education requirements to meet facility and regulatory agencies accreditation and licensure requirements.


 

EDUCATION

  • PHARMD-Graduate of an accredited pharmacy school

CERTIFICATION & LICENSURE

  • PHARMR-Current registration or Registered Pharmacist within 120 days
  • ASHP-Post Graduate Year 1 ASHP General Pharmacy Residency




TYPICAL EXPERIENCE:

  • 3 years recent relevant experience



SKILLS AND KNOWLEDGE:

  • Knowledge of medical terminology, generic and trade pharmaceutical names, pharmaceutical calculations and laws and regulations.
  • Knowledge and understanding of different Pharmacy practice settings.
  • Knowledge and ability to identify and employ pharmaceutical and medical terms, abbreviations and symbols commonly used in prescribing, dispensing, and record keeping of medications.
  • Possess written and verbal communications skills to explain sensitive information clearly and professionally to diverse audiences, including non-medical people.
  • Well-developed time management and organizational skills, including the ability to prioritize assignments and work within standardized operating procedures and scientific methods to achieve objectives and meet deadlines.
  • General knowledge of computer applications, such as Microsoft Office Suite (Word, Excel and Outlook), related pharmaceutical technology, EHR, and EPIC.
  • Prioritize assignments and work within standardized policies, procedures, and scientific methods to achieve objectives and meet deadlines.
  • Work independently, as well as be part of the team, including accomplishing multiple tasks in an environment with interruptions.
  • Identify, evaluate and resolve standard problems by selecting appropriate solutions from established options.
  • Ensure the privacy of each patient’s PHI.
  • Self-starter who can work independently without close supervision and who ensures management staff are kept aware of any issues.

Pay Range is $98.34 to $113.09 / hour

The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
