Position Summary

The Sepsis Transition Care Coordinator (TCC), under the supervision of a licensed professional (Registered Nurse), provides the necessary support to help patients navigate healthcare transitions, ensuring smooth and safe transfer between different care settings, like hospitals and homes or post-acute care facilities. They focus on coordinating care, facilitating communication, and addressing patient needs during these transitions, specifically the high-risk sepsis patient population at MultiCare Good Samaritan Hospital. Under the guidance of a licensed professional, the sepsis TCC also take responsibility for ensuring the continuity and coordination of patient care delivery by helping identify patient needs; assist in developing (in collaboration with the patient, patient family, inpatient providers and care team, PCP/Specialty Physician, and MultiCare Health System Sepsis Pathway Team) a Transitional Plan that addresses those needs; identifying and leveraging appropriate resources (within the parameters of established networks, programs, and policies) to implement the plan; and regularly monitor the patient’s progress according to the plan.

Requirements

  • One (1) year experience in a health care setting, preferably as a Health Unit Coordinator (HUC) or in a clinical role such as medical assistant, certified nursing assistant, EMT, or LPN

Pay and Benefit Expectations

We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $18.75 - $26.99 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant’s years of experience align.
