Job Description

The Sepsis Quality Outcomes Nurse works independently to review patient charts, evaluate treatment plans, and monitor patient outcomes. This role is responsible for tracking and reporting the utilization of evidence-based best practices, collaborating with multidisciplinary teams to enhance the application of these practices, and driving improvements in patient care and clinical services throughout the hospital. The Sepsis Quality Outcomes Nurse may be required to perform other related duties as assigned.

Key Responsibilities:

The Sepsis, Quality Outcomes Nurse will collaborate with the Quality Department, Sepsis Team, and multidisciplinary teams to:

  • Review patient charts and treatment plans in partnership with multidisciplinary teams to monitor patient outcomes.

  • Track and report the utilization of evidence-based clinical practices and identify opportunities to improve patient care.

  • Develop systems to identify specific inpatient populations according to evidence-based guidelines.

  • Identify areas for improvement based on evidence-based practices, regulatory standards, and accreditation requirements.

  • Analyze data to detect trends, gaps, and opportunities in high-risk, high-volume, or problem-prone processes.

  • Conduct chart abstraction and review for key regulatory reporting, including The Joint Commission, SEP-1 Core Measures, and other indicators.

  • Create tools to track and analyze trends in patient outcomes (e.g. mortality, readmission, SEP-1 compliance, etc.).

  • Design and implement new processes and clinical care initiatives to continually improve patient outcomes and achieve performance targets.

  • Communicate findings and provide feedback to physicians and nurses regarding compliance with evidence-based practices and outcome measures.

  • Conduct and report results of clinical outcomes and process monitoring; design and develop dashboards and innovative data visualizations to improve care.

  • Advise on the appropriate use of quality improvement tools and methodologies, such as PDCA, Lean, Six Sigma, and Statistical Process Control Analysis.

  • Apply performance improvement methodologies (e.g., Lean, Six Sigma, FMEA) to quality improvement projects.

  • Mentor and lead the work of others as assigned

Qualifications:

  • Registered Nurse (RN) with current state licensure.

  • Bachelor’s degree in nursing (BSN) required; Master’s degree preferred.

  • Minimum 5 years of clinical nursing experience.

  • Experience with quality monitoring and abstracting data from medical records required.

  • Existing knowledge of SEP-1 core measure is preferred.

  • Strong analytical, organizational, and communication skills.

  • Proficient in data analysis, report writing, and use of quality improvement tools (e.g., PDSA, Six Sigma).

  • Familiarity with EPIC preferred.

License/Certification/Registration Requirements

  • Valid California RN license

  • Current BLS certifications

  • Certification in healthcare quality (e.g., CPHQ) preferred.

Ages of Patients Served

This position will serve the adult non-OB and OB patients.

Salary Range:

$73.68 - $110.52 USD Hourly
