The RN Care Manager within the Geisinger at Home program plays a vital role in delivering patient-centered care to Geisinger Health Plan’s most complex members in a home-based setting. This position works collaboratively with physicians, nurses, and an interdisciplinary care team to assess, plan, and implement individualized care plans that promote optimal health outcomes.
Typical hours are 4 Shifts: 8 AM to 4:30 PM. 1 Shift: 10 AM - 6:30 PM
Rotation of Weekends & Holidays. Will be fully reviewed at interview.
This role is to support home visits to patients in Luzerne and Lackawanna County.
Your primary territory will be Luzerne and Lackawanna County but may need to cover other areas as needed. Company car will be provided.
Prior nursing experience in an Emergency Department, ICU or Home Health environment is preferred but not required
A minimum of three (3) years of RN work experience is required.

Position Details:
Manage a panel of patients based on geographic location, ensuring continuity and coordination of care.
Develop personalized care plans and coordinate services across the patient’s healthcare team to ensure comprehensive, patient-centered care.
Practice at the top of the nursing license, delivering timely interventions at the point of contact to prevent delays in care.
Engage patients and their support systems in meaningful conversations to align healthcare delivery with patient goals and preferences.
Delegate appropriate tasks to interdisciplinary team members and maintain strong communication across the care continuum.
Facilitate enrollment, routine, acute, and transitional care visits in the patient’s home environment.

Education:
Graduate from Specialty Training Program-Nursing (Required), Bachelor's Degree-Healthcare Related Degree (Preferred)Experience:
