Become the Vital Core of Our HIV Clinic Mission! - Referral Specialist HIV Clinic
Overview:
The Referral Specialist supports coordination of client referrals and follow-up care within the HIV Clinic under Integrated Clinical Services (ICS). This position plays an essential role in ensuring timely access to care, effective communication between clinical teams, and accurate documentation to support high-quality, patient-centered services.
Primary Responsibilities:
Receive, review, and process provider-generated referrals for specialty, diagnostic, and community-based services.
Schedule and confirm appointments with outside providers and ensure all referral-related information is documented in the electronic health record (EHR).
Track referral completion status and conduct follow-up communication with clients and providers to ensure continuity of care.
Communicate directly with patients using culturally responsive, trauma-informed, and respectful language, particularly when discussing sensitive health conditions such as HIV, STI prevention, and chronic illness management.
Collaborate with providers, case managers, and care coordinators to address barriers to care, such as transportation, insurance, or appointment preparation needs.
Work closely with insurance companies and community partners to secure pre-authorizations and verify coverage for specialty services.
Maintain accurate records and reporting to ensure compliance with ICS standards, privacy regulations, and quality improvement metrics.
Identify and escalate urgent or complex cases that require clinical intervention or multidisciplinary team review.
Participate in staff meetings, quality improvement initiatives, and ongoing training to enhance workflow efficiency and patient experience.
Serve as a key administrative support for providers in managing high-volume referral queues, including updates to referral templates, resource lists, and contact directories.

Minimum Qualifications:
Education/Training: Equivalent to the completion of the twelfth grade; AND
Experience: An associate's degree and one (1) to four (4) years of experience that demonstrates the ability to perform the duties of the position. (Equivalency 3-6 years of qualifying training and/or experience).

Experience providing professional customer service to diverse communities.

Strong communication, documentation, and organizational skills with attention to accuracy and follow-through.
Ability to work independently and collaboratively in a fast-paced environment.
Familiarity with electronic health record systems (Epic preferred) and patient referral workflows.
Demonstrated commitment to equitable and inclusive service delivery for individuals and communities impacted by HIV.
Previous experience working in a medical or health related environment.
