The Population Health Analyst will support the Manager of Population Health Operations, the Manager of Patient Care Transformation, and the Director of Population Health around practice transformation and population health initiatives. This will include managing data, aggregating reports, and analyzing and presenting findings for Saint Luke's Physician Group practices. Manage regulatory reporting for value based programs for 10 primary care practices and the integrated physicians group. Manage performance monitoring for Saint Luke's value based incentives and determine high impact actions for performance improvement. Act as resource for practice staff and leaders regarding regulatory requirements and clinical quality measure workflows (including HEDIS, PQA and CMS measures). Review Epic charts for documentation, submit charts to payors in accordance with plan program requirements, and identify care gap opportunities. Maintain documents and rosters provided from payor partners and manage the attribution related workflows. Assist in educating care providers and reviewing charts on documentation requirements and identifying areas for documentation improvement.
