POSITION SUMMARY:
The Boston Medical Center Geriatric Services, Home Care Program provides primary medical care and intensive nursing case management to homebound frail elders living within the City of Boston. The Nurse Care Manager position is a unique one requiring a variety of nursing skills in order to effectively provide nursing case management for a panel of complex, high risk, nursing home level older adults.

ESSENTIAL RESPONSIBILITIES / DUTIES:
Performs a variety of high-level nursing care management services through the development and use of geriatric nursing practice standards. Activities include conducting a comprehensive geriatric nursing assessment, developing and reassessing a plan of care which addresses the physical and psycho-social needs of the older adult; triaging day to day clinical issues; and providing patient/family education via telephone and home visits as well as coordinating care and services across various sites of care. Ensures that desired patient outcomes are achieved and that variances from established best practices are evaluated and addressed as necessary.

EDUCATION:
BSN
CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:
Licensed to practice professional nursing as a registered nurse in the Commonwealth of Massachusetts.
EXPERIENCE:
5 years of experience as an RN functioning in a model of care delivery system that included care management.
Experience working with Geriatric population is preferred.
KNOWLEDGE AND SKILLS:

keen telephone triaging skills to evaluate episodic acute and chronic clinical issues with critical reasoning and astute clinical judgment to expedite appropriate care given the complexity of our homebound, high risk population
knowledge of community resources
knowledge of managed care insurance programs
ability and willingness to make home visits to provide direct patient care as needed
strong communication skills working w/ patients, families and caregivers as well as an interdisciplinary team w/in BMC.
ability to develop professional relationships w/ various community providers
ability to multitask and be organized
confidence to facilitate problem solving and decision-making
confidence and ability to lead a clinical team
Compensation Range:
$104,000.00- $151,000.00This range offers an estimate based on the minimum job qualifications. However, our approach to determining base pay is comprehensive, and a broad range of factors is considered when making an offer. This includes education, experience, skills, and certifications/licensures as they directly relate to position requirements; as well as business/organizational needs, internal equity, and market-competitiveness.
