Job Summary: The facility, complexity level 1b, has a diverse teaching, research, medicine, and transplant hospital and an extensive, multisite ambulatory clinic program. The duties of this position include the provision of advanced patient care to Infusion Clinic including clinical double checks and order verification in the hematology/oncology/infusion clinic space along with completing formulary team PADR workload.

Functions of Position: The Clinical Pharmacy Specialist (CPS) serves as a mid-level provider with prescribing privileges as outlined in the individual's Scope of Practice. The CPS functions at the highest level of clinical pharmacy practice to provide comprehensive medication management (CMM). CMM is a patient-centered approach to improving patient health outcomes through optimizing medication use delivered by a CPP working in collaboration with the patient and other healthcare providers. A. Clinical Practice Maintains an expertise in Infusion treatments for patients in Oncology, Hematology and miscellaneous specialty areas through provision of patient care. Performs clinical reviews and adjudicates prior authorization drug requests promoting safe, cost-effective, and optimal medication use. Coordinates care for patients with other healthcare providers as needed. Provides pharmaceutical care for patients in the designated clinic(s) according to his/her scope of practice. This shall include ordering of laboratory and other tests necessary to monitor care, referring patients to appropriate health care providers as needed, performing patient triage functions and ordering medications needed for supportive care including antiemetic, bone marrow stimulating agents and intravenous fluid infusion when needed. May monitor and adjust anti-neoplastic drug doses in collaboration with Hematology/Oncology staff in accordance with protocols for patients receiving infusion therapy. Evaluates orders for appropriateness and drug interactions, allergies, adverse reactions, and duplicate therapy. Intervenes with physicians and other staff members prospectively and retrospectively for rational, safe, and effective drug therapy and documents interventions. B. Distributive Functions Clinical double checks and order verification will include assessment of labs and drug-drug interactions, with the pharmacist making recommendations for dose adjustments to therapy as needed. Ensure that Veterans meet required treatment parameters and recommended monitoring prior to next cycle of cancer therapy, contacting Hematology/Oncology providers as needed for clarification of orders. Serves as a Pharmacy Service resource on inpatient and outpatient pharmacy services including CBOCs. Oversees control and inventory of Controlled Substances. Reviews service connection for appropriate costs and payment and applies VA local standards for appropriate dispensing and filling. Assists with inventory control and appropriate purchasing practices. Accurately compounds, dispenses medications, and provides pharmaceutical care in accordance with departmental policies and procedures. Participates in maintaining quality control of pharmacy products. C. Administrative Continually identifies and implements strategies to optimize the cost-effective use of medications. Continually reviews economic impact and compliance with these endeavors, and provides input to Pharmacy Leadership. Identifies target areas, formulates and develops plans for implementation, trains staff, and provides documentation to assess progress for formulary management activities. Conducts prospective/retrospective drug use evaluations of high-use, high cost, or high-risk drugs. Assist the Associate Pharmacy Chief of Business and Quality Management in preparation and report analysis of data, make recommendations for corrective action, and develop education programs/interventions that would alter aberrant prescribing or work patterns. Serves as a resource between Pharmacy Service and all health care providers regarding formulary management Displays effective leadership behaviors including emotional intelligence when interacting with others. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (TRANSCRIPTS REQUIRED) OR Must be a graduate of a foreign pharmacy degree program that meets the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). (DOCUMENTATION REQUIRED) Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. (MUST PROVIDE A COPY OF LICENSURE) May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to the basic requirements above, applicants must meet the following grade requirements. Clinical Pharmacy Specialist Experience, GS-13: You must have at least one (1) year of experience equivalent to the next lower grade level (GS-12), which include but not limited to: Knowledge of professional pharmacy practice; Ability to communicate orally and in writing to both patients and health care staff; Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security; Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. OR You must have completed a 1 year Post Pharm D. accredited Residency. AND In addition to the experience listed above, the Clinical Pharmacy Specialist must demonstrate the following KSAs: A. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. B. Expert understanding of regulatory and quality standards for their program area. C. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. D. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. E. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: Work experience in hematology/oncology pharmacy Creditable Experience: To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice. Professional practice means paid/non-paid employment as a professional or unlicensed graduate pharmacist as defined by the appropriate licensing board. Residency and fellowship training programs in a specialized area of clinical pharmacy practice may be substituted for creditable experience on a year-for-year basis. The pharmacy residency program must be accredited by the American Society of Health-System Pharmacists (ASHP). A fellowship program that is not accredited by the American College of Clinical Pharmacy (ACCP) will need to have comparable standards for experience to be creditable (Professional Standards Board refers to the Deputy Chief Consultant for Professional Practice for the determination). Quality of Experience: Qualifying experience must be at a level comparable to pharmacy experience at the next lower level. Experience as a Graduate Pharmacist is creditable provided the candidate was used as a professional pharmacist (under supervision) and subsequently passed the appropriate licensure examination. Part-time Experience: Part-time experience as a professional pharmacist is credited according to its relationship to the full-time workweek. For example, a pharmacist employed 20 hours a week, or on a 1/2-time basis, would receive 1 full-time workweek of credit for each 2 weeks of service.
