As a Registered Nurse Case Manager, you’ll be the bridge between medical care and human comfort — guiding patients and families through their journey with empathy, skill, and dignity. You’ll collaborate closely with a team that truly values your voice, your expertise, and your heart.
Deliver exceptional, compassionate hospice care that honors each patient’s dignity and wishes
Assess patients’ physical, emotional, and spiritual needs — treating the whole person, not just the illness

Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Bachelor’s)
Current RN license in the State of Michigan
At least 1 year of clinical experience (hospice or home health preferred)
Compassion, flexibility, and a heart for meaningful work
Valid driver’s license, insurance, and reliable transportation (50% travel required)
Ability to lift, move, and support up to 50–100 lbs. as needed
