Position Title: Case Manager (First Response)

Classification: SCHADS Crisis Level 2 / Social Level 4
Location: Still Waters – Ashmore
Status: Full-Time, Permanent – Maximum Term (6 months )  

8am – 4pm Monday – Friday

We provide services and supports to adults, and families with accompanying children in the form of accommodation, case management, assertive outreach, financial assistance, community connection and referral services.

About the Role

The Case Manager (First Response) provides both over-the-phone and face-to-face support, including triage, interim response, and case management services to individuals and families across South-East Queensland.

This role supports people who are at risk of or currently experiencing homelessness through initial assessment, appropriate referrals, and the development and implementation of person-led case plans. The work is guided by person-centred, trauma-informed, resilience-building, and strengths-based approaches to achieve meaningful outcomes for clients.

How you will make an impact:

  • Deliver services in line with the National Homelessness Stream Model of Care, ensuring consistency with national standards and expectations for individuals, partners, and the wider community.
  • Provide timely responses to phone and email inquiries, including interim assessments and risk evaluations using appropriate tools (e.g., QHIP, AVHHTS, safety plans).
  • Respond to walk-in clients, conducting assessments and making appropriate referrals.
  • Build and maintain respectful, empowering relationships that encourage choice, self-determination, and active participation.
  • Provide case management support in accordance with The Salvation Army (TSA) case management models and standards of practice.
  • Conduct ongoing assessment of client needs, vulnerabilities, and progress from intake through to case closure.
  • Collaboratively develop and implement individualised case plans that address client goals, needs, and barriers.

What you will bring:

  • Degree qualification in Social Work, Welfare, Community Development, or a related field; or extensive relevant experience demonstrating high-level sector knowledge and skills.
  • Proven experience in a social services environment, including client assessment and case management.
  • Current QLD Working with Children Check (required as the position operates at a site that provides overnight accommodation to young people and/or supports families in-home).
  • Current Driver’s Licence.
