About the Role
The ED Care Coordinator through screening and recruitment in the Emergency Department (ED) is responsible for the delivery of timely and comprehensive assessment and care coordination of  patients with complex needs to facilitate the allocation of hospital and community resources in an efficient and cost-effective manner. Assessment and care planning are achieved in collaboration with patients, their carer’s and other members of the Emergency Department Clinical team. This role also includes recruiting clients onto the program in a community case management capacity, conducting comprehensive assessments, coordinating all aspects of health and social care needs, monitoring achievement of client goals, facilitating intervention from other health care professionals and ensuring appropriate linkages with hospital and community services.

Classifications for this position will be Allied Health Grade 2

($90,261 - $103,724 per annum, pro rata for part time).

The program has a base of services within St Vincent’s that currently support clients that are experiencing or at risk of homelessness. This service aims to build on integrating and better coordinating existing services within St Vincent’s hospital and the development of new services to meet the need of the target client group. 

Operationally the role has strong links with the Health Independence Program (HIP). HIP offers a comprehensive range of specialist clinics, services and rehabilitation programs, providing rapid response health management, short-medium term care coordination and health education. HIP enable clients to be the decision makers of their care, and support clients through self –management to achieve their healthcare goals. 

ALERT key target groups  include patients with complex needs who fall under one or more of the key risk factors; substance use, mental health issues, homelessness, disability and complex aged care issues, family violence.
This position is based at St Vincent’s Hospital Melbourne within the Emergency Department and also providing support and interventions in the community as part of an interdisciplinary team.

4. POSITION PURPOSE

Assessment and care planning are achieved in collaboration with patients, their carer’s and other services involved with the clients care within the organisation and community services. This role also includes recruiting clients onto the program in a community case management capacity, conducting comprehensive assessments, coordinating all aspects of health and social care needs, monitoring achievement of client goals, facilitating intervention from other health care professionals and ensuring appropriate linkages with hospital and community services.


5. POSITION DUTIES

  •  Early identification of ALERT patients in the Emergency Department.
  •  Maximize a patient’s functional independence through assessment and treatment planning, implementation and evaluation.
  •  Provide appropriate discipline specific clinical management, discharge planning & community liaison for all relevant patients.
  •  To engage with and provide outreach care co-ordination services for identified or referred clients of the Emergency Department post discharge, including ensuring holistic assessment and collaborative care planning to guide interventions
  •  Effective negotiation and advocacy with relevant service providers to ensure optimal and coordinated client care according to best practice principles
  •  Ensuring appropriate use of brokerage funds 
  •  Active participation in team interface processes, service development, evaluation and portfolio activities 
  •  Perform within the Health Independence Program (HIP) guidelines and specific service guidelines
  •  Ensuring all documentation including client records are completed and managed in a timely manner consistent with SVHM and Complex Care policies and procedures, legislative and regulatory requirements
  •  Contributing and participating in Complex Care and ALERT professional development (PD) programs and other PD that supports own learning objectives for this role.

6. INCUMBENT OBLIGATIONS

8. SELECTION CRITERIA
8.1 ESSENTIAL REGISTRATION, LICENSE OR QUALIFICATION REQUIREMENTS

  •  Allied health qualification relevant to ALERT and minimum 2 years experience in relevant acute/community setting.
  •  Eligibility for membership of relevant discipline association
  •  Current Victorian Driver’s Licence

8.2 OTHER ESSENTIAL REQUIREMENTS

  •  Experience with prioritizing patients and hospital demands 
  •  Demonstrated sound knowledge of the application of care coordination practice frameworks and principles, in the context of the current health care environment.
  •  Awareness of Department of Health HIP initiatives and National Health Reform initiatives
  •  Demonstrated ability to work autonomously and as part of an interdisciplinary team
  •  Demonstrated ability to effectively work with relevant service providers to achieve best outcomes for clients 
  •  Proven ability to effectively manage and negotiate solutions to complex problems
  •  Demonstrated commitment to and experience in the development, implementation and evaluation of program activities
  •  Highly developed interpersonal skills that incorporate the qualities of excellent communication, negotiation, organization, planning and conflict management
  •  Excellent time management skills, including planning, organizing and prioritizing of workload
  •  Current Victorian Driver’s License
  •  Participation in roster which includes weekends, Public Holidays and afternoon shifts.


8.3 OTHER NON ESSENTIAL REQUIREMENTS
 Specialised skills in one or more of the following areas: Aged Care, Substance Use, Homelessness, Mental Health or Chronic Disease
 Postgraduate qualifications in related field
