Key Essential Functions and Accountabilities of the Job
Leadership & Team Management
Directly manage a team of managers or specialists responsible for handling referrals.
Provide coaching, performance evaluations, and onboarding of new staff.
Cultivate a high-performing, collaborative team culture.
Maintain leadership accountability for ensuring timely and effective referrals.
Establish a regular cadence for team members to work referrals for each panel and report back to PCP/Medical Home Teams with data and analytics.
Engage PCPs to adhere to standard turnaround times for referrals.
Referral Operations Oversight
Supervise the entire referral lifecycle including intake, evaluation, processing, and follow-up.
Ensure timely, accurate, and appropriate placement or service matching.
Process and Policy Development
Create, implement, and refine referral workflows, standard operating procedures (SOPs), and best practices.
Ensure compliance with relevant regulations and internal policies.
Data Analysis & Reporting
Develop and track referral metrics, evaluate utilization trends, identify gaps, and prepare reports for senior leadership.
Use data to improve operational efficiency and service quality.
Stakeholder Collaboration
Work closely with healthcare providers, service agencies, HR teams, and internal departments to align referral processes with organizational goals.
Provide guidance on policies and new initiatives.
Quality & Customer Experience
Monitor service outcomes and satisfaction to ensure referrals meet the needs of clients, patients, and employees.
Support operational excellence.
Capacity Planning & Growth
Forecast referral volume, align resources and staffing, and develop strategies to scale operations.
Budget for departmental needs.
Continuous Improvement
Identify and implement continuous improvement efforts to increase referral retention and reduce wait times for specialty services.
Implement and sustain a referral acknowledgment process to ensure patients receive confirmation within 24 hours.
Additional Details:
Education, Certifications, and Licenses Required
Bachelor’s degree in Nursing, Social Work, Healthcare Administration, Business Administration, or a related field
Relevant Master’s degree preferred.
Experience Required
Minimum of 5 years of experience in referral, case, or care management, with at least 3 years in a managerial or director-level position.
Proven experience in team leadership, operational oversight, and management.
Knowledge of
Familiarity with referral software, EHR systems (e.g., Meditech), project management tools, and reporting platforms is highly desirable.
Knowledge of healthcare, social services, or organizational structures depending on the sector.
Proficiency with operational software and data dashboards.
Skills in
Strong analytical skills for monitoring metrics, evaluating workflows, and optimizing outcomes.
Excellent communication, collaboration, and problem-solving skills.
Stakeholder engagement and training capabilities for internal and external teams.
