About Us – Social Services

  • The Social Services team provides discharge planning to every patient who needs assistance with placements, equipment, services, social issues, etc
  • Each inpatient department is assigned a Social Worker. This includes Freeman West, East and Neosho
  • Our team collaborates with physicians, nurses, ancillary staff, patients, and their families to provide the best care possible

Schedule: Full Time

What You'll Do

The Community Health Worker (CHW) provides care coordination to address health and social needs, develops strategies for care coordination, and facilitates connections to appropriate resources to meet health and social needs of patients and their families.

Requirements

  • HS diploma.
  • Experience working in a community based setting/organization for 1-3 years.
  • Community Health Worker - State of Missouri Certification within 9 months of hire.

Preferred Requirements 

  • Associates degree.
  • Bachelor's degree.
