POSITION SUMMARY:

Community Health Educators are frontline health and educational workers who have a close understanding of the community they serve. This trusting relationship enables them to serve as a liaison/link/intermediary between health/social services and the community to facilitate services and improve the quality and cultural competence of service delivery. 

In the Cardiology clinic setting (Moakley or Preston) or in the community as requested by the Medical Director, work with the patient, their family/support, of the needed cardiology services.

JOB RESPONSIBILITIES:

  • Clinical Practice- Educate patients under the direction of the medical director
  • Consultation- Consult with providers on high risk patients
  • Community Education- provide training and support to community as requested
  • Research- collaborate with section principal investigator or the clinical research staff on research subjects
  • Quality Improvement- Educate patients on quality initiatives as directed
  • Other patient education duties as assigned

JOB REQUIREMENTS

REQUIRED EDUCATION AND EXPERIENCE:

  • Bachelors degree plus 3 + years’ related work experience (or equivalent combination of years of formal training and relevant experience).

PREFERRED EDUCATION AND EXPERIENCE

  • Nursing degree preferred

CERTIFICATIONS, LICENSES, REGISTRATIONS PREFERRED

  • RN license preferred

KNOWLEDGE, SKILLS & ABILITIES (KSAs)

  • Experience in related medical/social/testing issues, such as addiction, homelessness, sexuality, and refugee/immigration issues preferred.

Compensation Range:

$23.80- $34.38

This range offers an estimate based on the minimum job qualifications. However, our approach to determining base pay is comprehensive, and a broad range of factors is considered when making an offer. This includes education, experience, skills, and certifications/licensures as they directly relate to position requirements; as well as business/organizational needs, internal equity, and market-competitiveness.
