Job Title: Cardiac Rehab Coordinator
Location: Muskegon

Position Summary
The Cardiac Rehab Coordinator organizes, implements, manages, and evaluates the Cardio/Pulmonary Rehabilitation Program. This role ensures each component of the care process is delivered in partnership with the primary care physician and other providers within Trinity Health.
Essential Job Functions
- Implement assessment, planning, treatment interventions, and ongoing evaluation of the client’s plan of care. Collaborate with the care team, manager, and physician to ensure quality outcomes.
- Monitor and report vital signs including blood pressure, target heart rate, oxygen saturation, shortness of breath, and perceived exertion during exercise. Communicate concerns promptly and follow emergency protocols.
- Maintain accurate and current patient records, including assessments, goals, progress notes, education, and charge documentation.
- Provide education and instruction on cardiovascular disease, exercise progression, home activities, and lifestyle management. Document teaching and communicate with the health team.
- Coordinate patient recovery by serving as a liaison and referral resource to other providers and community programs.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Actively engage in patient care conferences, discharge planning, and interdisciplinary collaboration.
- Promote healthy living and model an enthusiastic, optimistic attitude.
- Support Continuous Quality Improvement (CQI) principles and strive for excellence in care delivery.
- Demonstrate professional interaction and effective communication with team members.
- Pursue ongoing professional development and continuing education.
- Represent the organization in community events and committees.
- Uphold the philosophy, mission, values, and vision of Trinity Health.
- Maintain competency in electronic medical record documentation per departmental policy.
Qualifications
- Licensed Registered Nurse in the State of Michigan or degree in Exercise Physiology.
- ACLS certification preferred and/or education in exercise specialty.
- Experience in Cardio-Pulmonary Rehab strongly preferred.
- Knowledge of cardiovascular disease, emergency procedures, nutrition, physiology, health education, and CAD risk factor management.
- Strong communication skills, both written and verbal, with the ability to work effectively with diverse individuals and groups.
