The Behavioral Health Unit Clerk is responsible for providing essential support to the nursing team by assisting with administrative tasks and direct care services for individuals supported. This role requires strong attention to detail, effective communication skills, and a commitment to ensuring the well-being and safety of the people supported. The ideal candidate will have experience in a healthcare setting and a compassionate approach to patient care.

Duties and Responsibilities:

Reporting & Documentation

  • Immediately report any changes in the condition of individuals supported to the nurse.
  • Document and report all accidents, incidents, and injuries of unknown origin (e.g., skin tears, bruises).
  • Report defective equipment, grievances, complaints, and any allegations of abuse or misappropriation of property to the nurse.

Vital Signs & Health Monitoring

  • Weigh and measure individuals as instructed.
  • Measure and record temperature, blood pressure, pulse, and respiration (TPRs) as directed.
  • Monitor oxygen administration and report findings to licensed nurses.

Personal Care Assistance

  • Assist individuals with dressing and undressing as needed.
  • Support individuals with nail care, including clipping, trimming, and cleaning fingernails and toenails (excluding diabetic residents).
  • Provide assistance with bowel and bladder functions, including toileting and maintaining scheduled toileting programs per the care plan.

Medical & Nursing Support

  • Administer medications and feedings via gastrostomy tube according to physician orders and under the delegation of licensed nurses.
  • Perform colostomy care as required.
  • Assist with lifting, turning, moving, positioning, and transporting individuals between beds, chairs, bathtubs, wheelchairs, and lifts.

Transport & Mobility Assistance

  • Assist in transporting individuals to and from medical appointments, activities, and social programs as needed.
  • Accompany individuals to off-site appointments and engagements as required.

Other Duties

  • Demonstrates flexibility and commitment to meeting unit and consumer needs.
  • Performs additional responsibilities as assigned.

Job Requirements

Education & Experience

  • High school diploma or GED required.
  • Licensed Certified Nursing Assistant (CNA) preferred, in accordance with state laws.

Skills & Qualifications

  • Experience in a healthcare or behavioral health setting preferred.
  • Strong attention to detail and ability to follow instructions.
  • Effective communication and interpersonal skills.
  • Compassionate and patient-centered approach to care.
  • Ability to work in a fast-paced environment and manage multiple responsibilities.

SUPERVISORY RESPONSIBILITIES: 

This position has no supervisory responsibilities.

PHYSICAL DEMANDS AND WORK ENVIRONMENT:

  • Must be able to lift a minimum of 10 lbs.
  • Must be able to pull a minimum of 20 lbs.
  • Must be able to squat, kneel, crawl, crouch, climb, and stoop.
  • Required to regularly stand and walk.
  • Vision requirements include close vision, distance vision, and peripheral vision.
