Job Summary: About the Position This position is located at U.S. Naval Hospital under Defense Health Agency located in Camp Foster, Okinawa, Japan. This is a Direct Hire Solicitation

Major Duties:

• Design timeline-based care protocols that ensure high-quality, cost-effective healthcare for patients and beneficiaries under case management.
• Track client progress towards goals, modifies care plans as needed, and advocates for clients' rights and preferences.
• Communicate/coordinate with appropriate parties including command and other medical teams within the guidance provided by DHA. Work with behavioral health clinic staff to ensure patient needs are met based upon the patient’s assessed condition.
• Involve family members as integral participants in treatment, aligning external support systems with recovery goals.
• Schedule and monitor medical appointments, therapy sessions, and support services and ensure continuity of care across multiple systems. Help patients navigate healthcare systems and access necessary resources.
• Collect and analyze data to identify trends and patterns and makes recommendations for program enhancements. Monitors, tracks and intervenes as patients transition between levels of care.
• Assess, develop and recommend local operating procedures to meet the unique needs of the clinic, MTF and population served to ensure access to appropriate interventions and levels of care.

EDUCATION: Bachelor's or graduate/higher level degree: major study in an academic field related to the medical field, health sciences or allied sciences appropriate to the work of the position. This degree must be from an educational program from an accrediting body recognized by the U.S. Department of Education(external link) at the time the degree was obtained. SPECIALIZED EXPERIENCE: At least one year of specialized experience equivalent to the GS-11 grade level in the Federal Service which includes Identifying high-risk individuals and initiate early interventions and treatment protocols; Inputting approved metrics, patient data, and/or clinical progress notes into prescribed databases; Providing and documenting effective multidisciplinary treatment plans; Managing/developing databases, perform data analysis; Developing presentations using current information technology and computers.
